When your patients with severe emphysema are experiencing worsening symptoms despite optimal medical treatment, including drug therapy, supplemental oxygen, smoking cessation, and pulmonary rehabilitation, then they should be evaluated for the Coil treatment.
“The disease trajectory in COPD is usually marked by a gradual decline in health status and increasing symptoms.” GOLD 2014 – Global Strategy for Diagnosis, Management and Prevention of COPD, 2014.
The following criteria can identify which patients should be referred for evaluation by a specialist for Coil treatment:
- SEVERE – your COPD patient has a primary diagnosis of Emphysema, GOLD Stage III / IV with severe hyperinflation
- STABLE – your patient is not a high exacerbator phenotype, and has not recently been hospitalized for a respiratory event (i.e. within 3 months)
- SYMPTOMATIC – your patient is complaining of increasing dyspnea, despite optimal medical management of his/her emphysema
Patients meeting these conditions may benefit from the Coil treatment, and should be referred to a pulmonary center where they can be fully evaluated to confirm their candidacy for Coil treatment.
